L.E.F.T.A. provides a wide range of training forms and reports used by most Law Enforcement Departments.
Recruit Training Guide Checklist (user defined): A checklist of required tasks that the recruit is to complete during their training period. They may include traffic crashes, domestic calls, traffic stop procedures, etc. Each department can change the checklist to include the type of calls/instances that it feels is required.
Daily Observation Report: This report rates the recruit on a daily basis of his/her performance. The report includes over thirty categories of evaluation recommended by the San Jose Model. Each recruit will be rated according to a rating scale which shows the most and least acceptable areas of performance. The report also includes documentation and comments for all of the areas of performance that were observed. L.E.F.T.A.’s DOR also display the Standard Evaluation Guidelines of the San Jose Module, making it easy for the FTO to adhere to the required rating guidelines.
Daily Observation Report Tracking Sheet: This report is automatically generated by L.E.F.T.A. when the Field Training Officer fills out the Daily Observation Report (DOR). Once the DOR has been approved and been submitted to the Field Training Coordinator it will be stored in a data file. Once the recruit completed a selected training phase, all of the DORs stored in the data file will be automatically transcribed onto the Daily Observation Report Tracking Sheet, simply by the click of a button.
Remedial Training Worksheet: This form is to be used to identify a performance area, which is below standard and requires remedial training. This worksheet form is not meant to replace the remedial training time record on the DOR, but is be used only after normal preliminary remedial training efforts have not been successful. Prior to implementation of this worksheet, time spent in remedial training from the DORs will be indicated on this form.
Call Tracking Log (user defined): This form tracks the type of calls that a recruit handles during his training. It enables the FTO to discover areas of training that have not been handled, therefore making it easier to determine what type of training/calls should be handled during the recruit’s current training phase. The call tracking log is user created as defined by each department’s signals/dispatch codes.
End of Phase Report: This report is filled out prior to a recruit’s advancement to the next phase. It indicates whether or not the recruit is to continue to the next training phase or if the recruit is to be re-phased.
Report Exemplar Cover Sheet: This report is filled out as a cover sheet to exemplar reports that are handed in for evaluation. The report could be one that is acceptable according to the department or unacceptable.
Probationary Recruit Bi-Weekly/Monthly Evaluation: Once a recruit has passed the field training program, law enforcement agencies generally place their employees on an additional probationary period. The Probationary Recruit Bi-Weekly/Monthly Evaluation is a modified DOR which evaluates the newly hired officer’s performance during his/her probationary period. This evaluation can be done on a bi-weekly basis or monthly.
